Something else you may want to look into is whether or not your GPU is overclocked compared to the 'reference' clock speeds.
We have seen in the past that factory overclocked video cards can be a little unstable and a little "de-clocking" can help a lot.
You do not necessarily need to run at the reference clock speed but maybe somewhere between reference and factory overclocked speeds.
Side note: it is unfortunate that GPU makers do not have a number on the box relating to stability at the provided clock speeds. It has become something of an arms race with regard to factory overclocking, GPUs with higher numbers tend to sell better.
