Performance issue, very bad FPS and FPS drops

I updated with the patch and it did nothing to fix the FPS

No performance improvements here. Fps still drops rapidly to 16fps in some areas and then picks right up again to 25-30fps in another and then back again.
Videocard: SAPPHIRE HD 7770 1GB OC
Processor: Phenom II X4 Quad @3.2Ghz
Mainboard: Asus M5A78L-M
Memory: G-Skill 8GB DDR3
Soundcard: SB X-Fi Xtreme Music
PSU: OCZ 620W

Still forced to play the game on Low settings just to make it even remotely playable at 1920x1080.
