I'm gonna go all out wild here but my estimate is that 95% or more of these people only have themselves to blame. They downloaded some shady trainer/hack which had a keylogger or likewise in it. "Hacked" accounts are quite unusual, they're just "stolen" because people are stupid enough to download shady things from shady sites.
Word of advice:
1. NEVER share your account information with anyone, espically not on public forums.
2. Don't download programs from a site that looks somewhat suspicious
3. Use a proper anti-virus software to shield yourself from malicious software
4. NEVER use the same password for multiple accounts/sites.
5. If an account gets comprimsed, immedeatly change passwords if you're using the same on anything else.
6. Change your password every now and then and be sure to make it something stronger than "hello123". Mix upper and lowercase, use numbers and things like !"##¤&%(%.
If you find yourself with your account being stolen or "hacked". Always contact customer service, preferbly by phone if there's an option for it and make sure you get in touch with them asap when you discovered that the account is no longer in control.
