Really bad vibration

I'm playing on PC with an Xbox One controller, 100% of the time that I'm ingame the controller is pulsating and while driving it has unexplained bursts of really intense vibrating. It's that bad that I'm genuinely concerned that this game will break my controller. There's no reason to have this 24/7 pulsating and random vibration feedback. 
There's nothing wrong with my controller, I use it on many other games with the vibration feature working correctly.
What's causing this issue and how can I stop it happening?
