Looking at the screen you attached, it shows 21ft to the hole and 1ft of uphill elevation.
To make that putt, you should have been giving 35 ft of power. If you only gave 4 yard of power (12 ft), then being 7 yards short (21 ft) is about what should be expected......

I respectfully disagree, because the shot began 8 yards ABOVE the hole. So if I set the landing at 4 yards (12 feet) there is no way (at least on planet Earth) that the ball will travel the entire 8 yards (24 feet) and end up 21 feet from the hole (7 yards) plus the other 4 yards that it traveled after the shot was set to land 4 yards from the hole.
So, altogether, the shot was set up to fly 4 yards, yet it went a total of 15 yards!! You go ahead and try to
make a 4 yard shot travel 15 yards when you hit it for four yards. It’s INCONCEIVABLE.

All I can go by is your screenshot. I had assumed the screen was from prior to the shot and you were describing what happened to the ball after you hit the putt. If this was the case, then my analysis was correct.
If however, the screen was a post of the -result- of the shot you took, then the screen shot doesn't really tell me (or anyone else for that matter) anything as we can't see what your pre-shot lie/distance/club used/shot type used/ etc are.
Based on your last description, I'm wondering if you were off the green behind the hole and you did a chip? A 4 yard chip for an 8 yard distance is the correct amount of rollout on a level surface. However, if the screen was the result of your shot, one can see it was a severe downhill slope if you were chipping from behind. It would be very conceivable that such a chip would continue to slide downhill......
