Sudden unlocked premium plants and upgraded to max levels

I've opened PvZ 2 and hit the Play button, then a moments later the game crashed and I open it again and this was the result. (See pictures below for the results) My gems and coins was turned from 4...

Greetings! I have been received an email from EA Help Center and here is the info. (See the picture that I attached below this reply.) 

They said that they need more info which is my Player ID and my Profile Name.

I have already my Player ID, my question is what Profile Name should I give? My Profile Name on the game or the profile name on my EA Account?

    I am sure they are asking for both the ID and Profile name from the PVZ2 game.
