Can someone tell me how to get a gnomish perspective hat item?

As for your question, as I understand it you need to find all of the gnomes then go to the secret room.

There are some short cuts but if you follow the video above you can knock it out fairly quickly. Choose private play, speed boost, and 0 gravity. Make it so there are no enemies or people on your team. Basically an empty game with just you in it.
