I've had this happen too, randomly, and with various characters. The controller vibration will suddenly be there, then not during game play.
ETA: Was playing the new Weirding Woods open section on an Engineer with vibration working. Spoke to a quest giver (Otto Do?), and after interaction was complete, no more vibration. Continued play as normal, without any resetting, some time later after several other quests/interactions vibrations randomly came back. Also, this may be tied to pixelation/graphics distortion which started up again.
