You jump through the hatch that opened up. When you get to the middle of that space, doors will open and that is where you will find Zomboss. Talk to him and you will achieve finder of doors and be on your way.
I believe the 5th badge is the LEAF badge which you get when you complete all of Dr Fizzician quests as well. He is the guy in the hole you jump in to the left of Dr Patient. Dr Fizzician has some more quests to do.
(6th is the delivery badge for delivering pizza from the burger place.)

Yes, I finished Yuck! and also did Dr. Fizzician ( three under the hatch. I have not been able to see Dr. Zomboss, just Dr Fizzician. The finder of doors quest doesn't show up on the board. Isn't it supposed to?
