Need to see more of your dump log but I have a pretty good guess.
I'd say your AMD/ATi drivers are corrupt. I'd uninstall them from CP, restart into safe mode, Run (Display Driver Uninstall -or- Radeon Removal Tool -or- Driver Sweeper(older)), restart, install drivers fresh.

Safe Mode w/ Networking if you get into trouble. Likely just Device Manager remove the display adapters restart.

You wont believe this, i deleted the memory dump from c:\windows so i could create a small dump because this was 500+mb. When i tried to replicate the crash it just worked, i closed the game and tried again and it worked again. Im doing this via team viewer from work so ill test more when i get home.
