Next time to you see it, take video and record details such as what server you are in, their gamer tag if possible, the time and date you were playing. Then go through the reporting process you have on your platform.
Also post this details here, minus the gamer tag. I will make sure the appropriate people see the post. Players glitching out of the map is an issue that is taken seriously and disciplined when possible.
