Having combed your Dxdiag again, here are some more suggestions:
- Install monitor drivers
- Remove any other attached hardware peripherals (ELAN input device?)
- Free up more hard drive space
- Have you performed any custom registry edits?
- What is DLLHost.exe? it is causing some issues
- Manually run all vcredist files from game install location

I will have to give these a try when I have a chance. (Later this week or next weekend).
Not sure if it is related, but event viewer is also showing a problem with atidxx64.dll (i think, I am not home right now) causing titan fall 2 to crash.
I will have to look into dllhost.exe and where it is installed, it should be a standard Microsoft program unless it got jacked.
Can't do much about the ssd space, it is only a 120gb drive.
No custom reg edits I can think of other than removing anything raptr gaming evolved related as per AMD's uninstall recommendation.
I will report back when I know more.

I can confirm dllhost.exe is installed under system32, so it is likely not been jacked by a virus:

Although it's file size is 21kb, so this doesn't seem correct.I can also confirm that atidxx64.dll is crashing Titanfall 2
