Thanks for attaching the DXDIAG report straight away. It helps.
Take a look at the Diagnostics section at the bottom of the report. There is some funky looking stuff to look into that may by contributing to your issue. You might, for example, disable/uninstall Discord. Also, while troubleshooting, I suggest you connect just one monitor.
If you've ever installed any game mods, that's possibly getting in the way.
