I suggest you attach a DXDIAG report, as someone may spot something.

Here's some things to try:
- update to the latest NVIDIA driver
- keep the 4K monitor unplugged for troubleshooting
- repair and keep the standard files instead of modifying BootOptions and ProfileOptions_profile - modifying is usually not "the" fix
- make sure you're running a release of Windows 11 that is not a Windows Insider (e.g. beta) release. I've seen some funkiness there lately.
- try a different cable and port with the 1080p monitor
- turn off all overlays (e.g. GeForce) while troubleshooting
- see if your video capture devices are getting in the way. for this matter, unplug all devices not directly needed to play the game (while troubleshooting)
- figure out what's going on with the errors showing up at the bottom of the DXDIAG report
