Before I'll send it our team for further investigation, could you check a few things for me? Since your friends do not experience a similar problem on their consoles, we need to do a few troubleshooting steps.
Could you try to power cycle your Xbox and restart your console while keeping all apps and games first to see if it will help with your issue?

I've done as you said and the bug is occurring less frequently, once every 3 games rather than every game. Like I said earlier it has been happening since patch 2.0. Just for further information I'm not experiencing any other bugs.
