I definitely do think that the lobbies occasionally bug out and will not launch a game regardless how many players are there. I haven't experienced it regularly but it has been there since launch. Yes it does happen as @RogueZeroRendar stated when people are loading etc. But there are times you can wait and wait because its bugged out. I know this as I have sat in a lobby missing one person where my team has 3 people with heroes /villains selected. The final person joins selects theres and it still says one player required sat there and waited for at least three-four minutes with no one leaving and still it doesn't launch. Obviously after about three-four minutes people from my team left. Now if that's the lobby not showing one of my team having left three minutes prior its not what I would call working correctly. But Im not sure it can be as on one occasion I'm thinking of I recall the first person that left had started doing that frustrated scrolling through heroes selecting and deselecting. In any case I can't help but think that a lobby system more like SWBF1 would go a long way to solving quite a few problems.
I'm not used to Console playing, so yeah it's a shame you can't contact the opposing team to check on that.
But a CTD (Crash to Desktop) may also occur in a Console, as a simple Crash to Console Menu. Crashes can take eons to be processed by the server. And sometimes even the console can be going through some issues and takes ages to load, to which the server has no time-out feature to force a maximum loading time.
This all could be solved if the message for when there are only loading players delaying the start of a match was something like: Waiting for # Players to Load
