One such case I managed to reproduce was palpatine's Dark Aura making Anakin stuck in Heroic Might if the aura damages Anakin as he's about to release his Heroic Might.

In GA it can be a death sentence indeed, but if you manage to get an enemy villain near you, you might be able to get unstuck if he manages to saber attack you, melee you, or use an ability which staggers, immobilizes, or causes a forced movement effect. I believe besides Bossk and Boba, all other villains have at least one such move (as it's highly unlikely a villain will melee you).
In HvV OTOH, it's far easier to get out of this BUG without the need to die/respawn
