I have recognised exactly the same, guess it's caused by the heavy server lags showing you the wrong position on the current map. Try to fly in GA, it's (almost) impossible.
I had this happen in SA yesterday, on PS4. There was obviously some significant lag, as other players were rubberbanding all over the place. So one moment they were in one place, then all of the sudden in another. So I think I was "teleported" into an object so a collision was detected, even though there had been nothing around me just before I died.
