I think it's valid to point out he attempted to use the rush skill (right skill) just before he died. This seems more evidence that are these skills that cause this bug.

DRAX Gaming uploaded a video where this happened to him with Phasma and her Staff Strike ability. Here is the link with the correct time to see it: https://www.youtube.com/watch?v=jRLtnnU9l8U&t=55 (if it doesn't work, it's a top5, and the one mentioned is in the 4th spot)

To me all this is kind of connected: these kind of abilities that apply forced movement + damage somehow don't stop after the character dies, only when the player respawns.
