You did turn off DX12, right?
Try unplugging one of the monitors and see what that does.
You want to turn off any overlays for troubleshooting.
There are multiple errors at the bottom of the DXDIAG report, including something about a wallpaper32.exe. I'm not well versed in those error codes, but something in that section may be contributing. There's also something in that section that suggests you have/had mods installed. Good luck there. You may have a system reset in your future.

While the mod files may not be the issue, it is certainly a possibility. I suppose a related question to ask yourself is, "Did I install mods between the time the game was running fine and now?"
I gave you some examples of things I saw at the bottom of the DXDIAG report. I suggest you look at whatever else is showing up there.
