Yep, I've had this happen randomly about 10 times as well.
Happens to me quite a lot lately, that the scan dart seemingly is out of cooldown but cannot be used nonetheless. Seems to be a bug though. Wouldn't make much sense for it to be a feature.
I saw this with an enemy scanner last night. You see the blinking scanner last for a very long time, much longer than can be accounted for by the card upgrade that makes it last longer (I use that card myself, so I know how long it should last).
