Yep, I've had this happen about 50 times, the only way to fix it is for your character to die and restart, or to quit the game and restart.
I don't think EA are going to bother fixing this, it's been happening for months now.
Lots of people reporting this in various threads. I've noticed that this almost always happens to me after dying while in scrambler infiltration mode.
