I had exactly the same thing occur to me on PS4 last week, but quitting it 'fixed' it and I haven't had it happen again since.

  This bug has got worse. Usually it's on 1 map that gets affected the whole night I play but tonight it happened on 3 maps. So basically when this bug happens u can't play any of these maps all night!
