Audio keeps going out!

How often does the bug occur? Often (50% - 99%)
Steps: How can we find the bug ourselves? No idea, it just started happening to me after a patch was installed. I just walked around and the soundtrack starts getting chopping, skippy or disappears as a whole. In some cutscenes dialogue has just dropped out entirely.
What happens when the bug occurs? The background soundtrack becomes chopping, like a skipping CD or just drops entirely. Dialogue has fallen out of sync of disappeared completely at times in cutscenes.
What do you expect to see? I expect smooth sound and full dialogue to play

This has completely ruined my play experience with this game. I could deal with some of the other buggy stuff, figuring you guys would get to it sooner or later, but audio issues like this are just god awful and breaks everything in the experience.
