Game Glitched? Missing Databank Entry

I completed the game a few days ago, and I'm now using Youtube to go back over the game and find all the missing databank entries and chests.  One particular entry I remember doing, but it doesn't show up in my list.  What's worse, the location where the entry would be made no longer shows an option to scan.  So now I'm never going to get 100% on this, because I can't scan it again.  I'm not sure what caused the glitch, but if anybody can help that would be great.

The specific Databank entry is on Ilum, listed as "The Gathering", entry #2.  I didn't get it on the way in, but I got it on the way out.  It was a force echo, where a Master was leading a Padawan through and the Padawan was upset because he was lost.  I remember getting it.  I just don't know why it doesn't show in the Databank and I REALLY don't know why I can't get it again.

Is there any way a tech can access my account and see what's up?  Maybe have them play my guy and see that it's not available?  I guess I could set up a tripod or something and record a video of it, but I'm not sure all that's necessary.
