Gamebreaking Glitch

Summarize your bug I was trying to get through the brotherhood village on Dathomir and found an improperly coded ledge. If you are close to the gate that leads to the bridge (the blocked off door) jump on the wall under the raft style ledge to your right if looking at said door. The jump up with double jump and hold LT. You grab on to the ledge and climb up. From that point you can fall outside all game bounds and even through the entire map itself if you find the right place. I found it because I was frustrated on trying to go to the other side of the blocked door.
How often does the bug occur? Often (50% - 99%)
Steps: How can we find the bug ourselves? Find the place that I described and make that ledge ungrabbable.
What happens when the bug occurs? You are put in a place that you aren't supposed to be and can fall through the map
What do you expect to see? I expect to see that the ledge is like any other ledge that should not be grabbed. You should bounce off
