Getting captured and returning to Ordo Eris after Kyber breaks BD-1 & the Mantis

Product: Star Wars Jedi: Fallen Order
Platform:Microsoft XBOX One
Summarize your bug Multiple related bugs appeared after losing an encounter with Pango Two Teeth: First was a total loss of control of Cal, then BD-1 cannot be retrieved, and finally it is not possible to even travel to another planet
How often does the bug occur? Occasionally (10% - 49%)
Steps: How can we find the bug ourselves? - After completing the Kyber stage, use the holotable travel to Zeffo* - Head to the caves found to the right of the Mantis, past the Stormtrooper getting devoured by the Scazz - Slide down to the lower cave, then head to the right of the landing area, where Pango Two Teeth should be present - Fight against Pango Two Teeth, and get captured - Further issues are encountered upon progressing through the already completed Ordo Eris and completing the gladiator fight
What happens when the bug occurs? First: Cal speaks a few lines wile the level is finished loading, before regaining consciousness. When Cal regains consciousness, he cannot be controlled. Exiting and returning to the game will return control of Cal. Additionally, the Tomb of Miktrull datacard will show on screen, even though it was already completed 2nd: BD-1 can be seen in the cell where we first retrieve him. However, he does not respond to any player input, and entering his cell effectively traps the player completely. 3rd: Once the arena fight is completed, the Mantis will land. Entering the Mantis will trigger a short black screen, then we are inside the Mantis. The outside of the Mantis shows outer space, none of the seats or NPCs are loaded, and the player cannot go past the potted plants. Interacting with the galactic map doesn't show any of the planets, and does not allow travel to the other planets. Furthermore, the player cannot exit the map once interacted with.
What do you expect to see? 1 - Player has control of Cal upon regaining consciousness 2 - BD-1 is either not in a prison cell, or responds to Cal's presence 3 - The Mantis is correctly loaded, and the player can travel to another planet

Notes:
- This was encountered on my first run of the game, which I'd bought last week.
- Possible extra steps to repro:
- When travelling to Zeffo, save and close the game after landing
- Exit the game, then return, and continue the session

I've compiled the results of this bug into the following, in order to better illustrate the issue
