Yours says "Driver Version: 26.21.14.3648", when it should say "Driver Version: 26.21.14.4120"

i would see if any change with a clean install of the video driver , select custom install and tick clean install
the dxdiag is showing crashes with nvwgf2umx.dll and Bccode 141
your running 32 : 9 ? not sure the game supports that would be my first question, have you tested a different monitor just in case ?
