Your dxdiag is showing store corruption , to sort that open a command prompt as administrator and type sfc /scannow and let that run

the dxdiag is also showing multiple bccode 141 , you could use DDU and then do a clean install of the video driver just to be sure the video driver is ok

Before your reply I went ahead and used DDU to completely remove and reinstall the video driver. After this fix, I still experienced game crashes ever 5-10 minutes.
I have just run the sfc /scannow in CMD and it completed stating that "corrupt files were found and successfully repaired. I will run the game and see if this worked.

Is there any response of this issue. I cannot play this game for more then 10 minutes before it crashes.

Try selecting the repair option in the Origin launcher, it worked for me.

I am having the same issue. Game will not run for more than 5 or 10 minutes before crashing. To be more specific, it has never happened directly at launch, and I don't think it happens during cut scenes (although it may have once or twice, I can't remember at this point). It usually happens totally at random during gameplay; sometimes in combat, sometimes when I'm just exploring, and other times when I'm just sitting there. I keep getting the errors LowLevelFatalError and LoginId error. I tried pretty much every fix I could find on the internet: Updating GPU drivers, repairing the game via steam/origin, setting GPU to factory clock speeds, not running programs like MSI Afterburner at all to minimize background processes, I tried underclocking my GPU, locking the frame rate to 30 fps, I cleared my shader cache (after updating drivers), etc. There was one instance where I was able to play it for 2-3 hours with no problems. One of my ideas was that maybe it has problems with Steam, so I uninstalled the game and reinstalled via Origin. Right after that, I was able to play completely uninterrupted for 2-3 hours and it didn't crash, I quit the game normally when I was done. Went to play it again today, and the crashes are right back; that's when I updated my drivers and cleared my shader cache. The only "fix" that seemed to work (temporarily) was a clean install via Origin. I don't know what started making it crash again. Amazing game, it's a shame it's nearly unplayable on PC; wish I got it for xBox.
