PC Version of Game freezes with Dual Shock 4 Controller plugged in via USB

This hadn't been a problem previously, but when returning the game after a few months away (and an update or two), the game now freezes shortly after launch (always just when the EA logo is starting to fade). If I unplug the controller, the game resumes and proceeds to the menu screen normally. If I plug the controller back in, it will freeze again, usually followed by Crash to desktop. I'm not seeing any error messages.

Drivers have been updated, and I've both repaired my installation and uninstalled/reinstalled - the issue persists. I've tried with Steam running and closed, as well as DS4Windows. No change.

Any help or hints would be appreciated!

    Did you try plugging it into a different USB port? USB 2.0 is typically the most reliable. It works fine on M+K?
