From personal experience, it seems like when you start to play a PS4 game on a PS5 and later switch to a PS5 version, the trophies don't carry over. Every other trophy needs to be unlocked first of course in order to be able to unlock the 100% Platinum trophy. This is unfortunately not something that we can fix, as this is how the PS4 > PS5 versions seem to work. When playing the PS4 version again on your PS5 and completing the game to 100%, it should unlock the trophy.

On various forums (example), I can read that for a small number of players on the PS5 everything was okay in regards to trophies, but for a lot, it wasn't working. It could be that it will unlock with a slight delay, after a console reboot or a hard reboot. But unfortunately, it's not guaranteed.
