Your dxdiag is showing store corruption , to sort that open a command prompt as administrator and type sfc /scannow and let that run

the dxdiag is also showing multiple bccode 141 , you could use DDU and  then do a clean install of the video driver just to be sure the video driver is ok

  • Before your reply I went ahead and used DDU to completely remove and reinstall the video driver.  After this fix, I still experienced game crashes ever 5-10 minutes.

    I have just run the sfc /scannow in CMD and it completed stating that "corrupt files were found and successfully repaired. I will run the game and see if this worked.
