Steps: How can we find the bug ourselves? In the Second Sister fight (I believe it's about 34-36% of the way through the game in case there are more Second Sister fights), I came down the elevator. This wasn't my first time fighting her, so no dialogue. I stayed in the elevator and force-pushed her, which I guess initiated the fight. She came into the elevator, I left the elevator and the doors closed behind me. I couldn't open the elevator doors from outside the elevator and I guess the Second Sister couldn't open the doors from inside, so she was stuck and I had to quit and restart.
What happens when the bug occurs? The Second Sister got stuck inside the elevator, meaning that I couldn't continue because I had to fight the Second Sister to continue, and I couldn't do so with her in the elevator.
What do you expect to see? I expected to see either the Second Sister unable to enter the elevator or the elevator doors open because she was inside.

While fighting the Second Sister at about 34 to 36 percent of the way through the game, I came down the elevator, force-pushed the Second Sister, which initiated the fight. She came into the elevator and I left with the doors closing behind me, and she got stuck in the elevator.
