Can you start with the game pad, go back into options and control and then re specify mouse and keyboard?

If that doesn't work @EA_David do you think the trick that worked with DA:I would also work here since it seems to be the same problem?

similar issue here but keyboard and mouse will not input commands into the game

  I'm having a similar issue as well! Except I have Nvidia GEFORCE and my Intel is CORE i7!

    found that if you disable any other display others than your main this resolved the issue for me.
