I can see in the DxDiag (thanks for that btw) that there's a program on your PC that is crashing quite a bit, which could potentially cause these issues (together with SWSD) for you. The program is called " DTHtml.exe", which seems to be related to ImageTune from Portrait Displays.
Do you happen to know if you have this installed? If so, could you please try to turn this program off and check if the PC still crashes?
It does appear to be installed! It's a program that appears to be tied to my monitor. How do I turn it off though? It doesn't appear in Task Manager - I found it through a file search using Control Panel.
