The trophy does not unlock: A Galaxy Far Far Away

Product: Star Wars Jedi: Fallen Order
Platform:Sony Playstation 4
Summarize your bug I fully explored all the planets but the trophy did not unlock for me (ps4)
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? I fully explored all the planets but the trophy did not unlock for me (ps4)

For anyone who has this problem:

I accidentally solved this: close the game completely (or turn off the console), then use the ship travel to each planet. Don't use the hologram on the ship to check the progress (it shows 100% but incorrect), open the minimap each time (do not use hologram on the ship) when you land on each planet to check your progress, it will show you the true progression.

    I am having the same problem except on Xbox One. 

    I am gonna test out your method later and see if it also works for the Xbox One achievement.

    @Kevinliufan1 Ya man. I did it. As you say the map on the ship is unreliable. So I have to check through every section of the area on each planets and found a place I have not explored fully. Finally I did it and get the trophy. Phwe love you man 🙂

    @Kevinliufan1 How fix the problem? I have 99% zeffos hologram and I think that the missing 1% is by a red door in the arena when Cal fight with The Second Sister. I need help :_(

    I'm having this problem so should I go to each world and open the map when I leave the ship and step into each world
