I fully explored all the planets but the trophy did not unlock for me (ps4)
I think I accidentally solved this: close the game completely (or turn off the console), then use the ship travel to each planet. Don't use the hologram on the ship to check the progress, open the minimap each time when you land on each planet to check your progress, it will show you the true progression.

Not working for me either. It is very frustrating considering this is the last one that I need for the platinum.

It is not working for me, I have the explored check mark under the planet name after I visit each one. Had turned everything off and revisit each individual planet and trophy does not pop up. HELP, its the only one I am missing for the platinum. @EA_Atic

Only the trophy "In a galaxy far, far away" was missing.
I was 96% in Eilram's tomb.
The trophy appeared opening all the air gates in the tomb.
