This same bug, verbatim as described above, is also happened to me right now on PC.
Even switching out the dual stance at a meditation doesn't fix it.
Sorry to hear you’re experiencing this same issue on PC. Yeah, unfortunately mediation points don’t fix this bug once it takes effect. The only solution is to exit out of the game and load back in. Hoping they fix this with an upcoming patch.
