Authenticator troubles

Recently, when trying to login, I have to input my authenticator code multiple times before it is accepted. I thought it might have been an issue with the launcher, but I've just tried to login to the store, and have had the same problem. I'm using the mobile authenticator for Android. Anyone have any ideas as to what the problem might be? Should I re-install the app? etc

Thanks for the help folks, but I think I found the answer a couple of pages down. Apparently my phone was set to manual time and date, and once I put it back to auto and synced it, everything returned to normal. Strange that being out of sync by even a little bit can cause so many problems.

Update : I've gone into my account, and removed the Authenticator and then uninstalled the app. I've tried to setup the app again, but this time, none of the codes are recognised. I've made sure I entered the serial number correctly, so now I'm out of ideas. Anyone???

No Idea what's causing the first problem, but the second is probably because you use an old version of the app.

Uninstall the app from your phone, go to the google play / app store and redownload it. Not sure whether you have android or iPhone but on Android the last update should be June 10, 2015 which is the one you want to have.
