If you are having issues with the cs via phone or ticket, I would recommend contacting customer service via phone and ask to speak to a supervisor or request one via the ticket system. As per BW here
If you feel you have been mistreated, or you have an issue with a Customer Support Rep you have two options:
- Call Customer Support and request to speak with a supervisor
- You may reach out to the community team with details: communitysupport@swtor.com.
Been having issues with customer service tickets repeatedly...the last customer service rep I spoke with patronized me flat out.

If its the same customer service agent or a group of them, note their avatar or in-game names. Then you can email that information to the email above or call and speak with a supervisor about this. This would help them with whatever they do to handle it.

Will that work? Because one of them gave me a big spiel about how the title had been removed since beta which wasn't true then patronized and suggested I could be violating the tos for cursing even though it's third time I've brought the issue up and still no helpful response at all.

As far as getting the title I can't guarantee that, you would have to go through the channels. What I was suggesting is you can bring up to a sup or via email how you were treated if they treated your poorly. Whether that will get the title granted to speak to a sup or email I can't say.
I did some searching on the forums at swtor.com, from what several threads that talk about it. The Oldest one being here with the first post from 12/17/2011 so it may have been removed in beta not sure. Other threads mention it being removed since 4.0 came out so not sure on that either.
