Lost piece of armor when I unequiped it
I unequiped my head gear, and it wasn't ging to my inventory, it was just gone.

EA QV TeamFirst thing would be to check the vendor's buyback tab to see if you accidentally sold it. This would be if you un-equipped by right-clicking. If you un-equipped by dragging and dropping, you may have accidentally destroyed the item.
