Can you provide a dxdiag per this link
also with the game closed go to C:\Program Files (x86)\Electronic Arts\BioWare\Star Wars-The Old Republic\swtor and dleete just the file called diskcachearena then try again

Deleting the requested file also made no difference.

There are some crash reports for windows update and your sound driver
for those open a command prompt as administrator and type sfc /scannow see if that finds and fixes some stuff.
I dont think it would be related to this swtor issue though, i gather you have tried window mode etc too
what is your swtor username in case not linked to this ea account , and will flag this to see if CS have any thoughts on this and if a ticket needs to be raised to look deeper for you

I dismissed my companion before entering the phase, but it appears on it's own again after I enter the phase. I've dismissed it once in the phase and tried it to start it and it didn't make a difference. Tried changing from PVE to PVP instances, story difficulty(story, veteran, master), fullscreen to windowed, various resolutions, removing character/companion clothing(cuz apparently that's a fix for a different bug).
I also ran the scan you mentioned, but it did not find any issues.

I tried using Mako, Blizz, Skadge, and 2V-R8.

I have more than one. I've begun questing on Corelia, and have the class quests for that planet.

I see that the quest is indeed no longer in my quest log. Do you know if I will have to go back and find it and complete it?
