have you tried booting with one monitor attached or attaching a different monitor if you have one ? (there is a reason to check this)
i tried unplugging one of my monitors and launching the game like that, but did not do a reboot with only one plugged in. i'll try that now.

d3d9.dll is really directx , so i guess cant hurt to install https://www.microsoft.com/en-au/download/details.aspx?id=8109

The crash reports have changed and Asus now appears too , maybe worth uninstalling the AI suite as a test ( think that is part of it )
KeyBotII.exeyou can press the windows key and type reliability and then open the reliability history , that can show you what is occurring each day

so uninstall AI Suite 3? and whatever that keybot is?

the only things in the reliability history is SWTOR many times, the keybot a bunch of times, and Origin once. i will uninstall the keybot and ai suite and try the game again.

did you have the issue prior to using 471.11 ?
league of legends for example is a d3d9 game of similar ilk to swtor interesting it is listed as failing to launch
maybe worth using ddu again and reverting to a 469 version video driver to be sure

i haven't played SWTOR in a couple of years, and am pretty sure it has never been played/installed on this machine before. i just recently felt like getting back into SWTOR and this problem came up from the beginning a couple days ago.
I have not had any problem like this before with anything else.

i'll try reverting to an older video driver. it's past my bed time already anyway.

i tried two different older video drivers and still have no change when i try to launch the game. went back to current video drivers.

So i finally got the game to load after formatting all drives on my machine and reloading windows from scratch.
Unfortunately i'm having a new issue with the game that i can't figure out now. it launches perfectly fine but when i get into the menus, the character on screen is extremely squished. it looks as if the resolution is way off. what throws me off is the UI looks normal like its correct. its only the actual game graphics that are really stretched out.
I've checked all the settings i could think of. my resolution is correct in game and on windows. like i said, i just did a full reload of windows last night so all my drivers were done as well and are up to date. is there anything you know of that might cause this to happen this way?
I'll attach a screenshot of what i'm talking about with the stretched graphics.
