Would you happen to be running Windows 10 and recently updated? I checked my Windows 10 log and there was an update yesterday. I didn't have this problem at all prior to today.
I opened a ticket and sent them my DxDiag.txt. If they respond I will make sure to update what they say on my post.

I tried to run in a different compatibility mode; still didn't work.
