2 & 3 are the only options. Technically #1, but you have to satisfy #3 first, so it's "more correct".
Option#2 occurs anytime you have the mod management set to "Advanced". Switch back to "Basic" and you'll only see red dots on mods that are able to be sliced (option #3).
