Same issue. Took Sim A to a few community lots while Sim B was at work. On the loading screen going home, the game froze. Sim A was missing from family on reload. Sim B's memories of Sim A replaced with '$Subject' with no headshot.
(After accepting the loss I remade Sim A, merged them back in to the family and rebuilt their relationship. My head-canon is they went missing and came back with amnesia, soap opera style)
