Had the same Bug where I took my newly adoptive child to the clothes store. It was going great. I bought new clothes for the child and as soon as my sim gets into the car with her new child it then goes to the loading screen that then takes me back to the home lot and the child doesn't seem to exist. I looked at the family tree and my sim has an icon with her face repeated as her own child. I checked to see if the child was in the Sims bin and they wasn't there either. I wish the sims will patch this bug out soon or I'll just have to make my sims hermits and never leave the housing lot.
