Romantic reputation is unpredictable
I knew that sometimes your sim's partner will accuse them of cheating when your sim hasn't actually cheated. But it also seems that some sims are utterly clueless about when they're being cheated on.....

Unfortunately, the Eternally Faithful reputation helps sims get away with bad romantic behavior. Not my preference, but it seems to be changeable from files I poked around in.
I think minor flirts might count, though, which could explain the other issue.
The related systems are far from perfect, but can be fun. o:)
