Another issue (because my game hates me) I found...
My one teen's mother scolds her for setting b o o b y traps she never really sets and tells her to go into time out. Then after a few minutes the mother goes to scold her only to RE-PUNISH her and tell her to go into time out again, resulting in an endless loop of time outs and scolding.

Edit cause it starred out "b o o b y"...
Edit again because I noticed this only occurs when the teen is having a mood swing.
