It's an active career—you need to be watching them all the time and telling them to respond to fires when applicable. At the top level, the sim gets a truck and fire alarm for home so they can stay home and wait for calls.
Sims gain experience in the career by responding to small and large fires and handling certain events in rabbitholes. They also gain experience by building their Athletic and Handiness skills, which they get credit for even if they do the skill building at home. So it's best NOT to have the sim build the skill before joining the profession—you'd want them to get job XP for all that work.
The part about responding to fires is partially bugged in that sometimes the sim will arrive on the lot and there's no fire. The theory is that sims already on the lot put it out. The way to get around this is to have the sim go to the sidewalk or street next to the lot, and only step onto the lot itself at the last minute.
